Privacy Policy

 

 

As a chartered physiotherapist we are committed to protecting your personal information. This privacy document relates to our use of your personal information that we have collected from you either in person, via telephone call or e-mail. Personal information relates to any information that is capable of identifying you. The collection and processing of data is a requirement of our legal obligation and it is relevant, adequate and limited to what is necessary.

 

The following information is collected from you directly:

  • Name and Contact details

  • Date of Birth

  • Medical Insurance details

  • Email address

  • Occupation

  • GP name and address

  • Consultant if appropriate

  • General medical history and current medication

  • Letters from you Consultant and MRI reports relating to your current condition

  • Clinical notes documenting your physiotherapy treatment

  • Musculoskeletal questionnaire – an evaluation tool pre and post treatment

 

 

How your information is used:

  • This information allows your physiotherapist to understand your medical background and factors that might relate to your symptoms

  • To provide a legal record of any treatment or advice we provide

  • To communicate with your private health insurance company

  • To communicate with you regarding appointments and to send exercises by email

 

Who might we share your data with?

  • Your GP

  • A Consultant specialist

  • Allied Health Care Professional - Podiatrist where clinically indicated.

 This information may be passed in the form of a written letter, which may be given to you, in which case it becomes your responsibility, and the protection of its contents is your responsibility.

If the information is passed electronically by e-mail, it will be password protected or will be sent via secure e-mail. We will take all reasonable precautions to transmit the information securely.

 

We will not pass your information for commercial purposes.

 

We take all reasonable steps to ensure that our information is kept up to date and rectified if necessary. It is your responsibility to inform us if any of your personal information changes.

 

How do we store your information?

 

Paper documentation with patient identifiable data is stored securely as current or archived.

Electronic data is encrypted and password protected.

 

How long we keep personal information for?

We have a legal obligation to retain records for 8 years after the conclusion of treatment.

If the records relate to a child or young person, these records must be kept until the patients 25th  birthday or 8 years after their 18th birthday.

 

 

Data breaches

 

Procedures are in place to detect, report and investigate any data breaches.

 

Requests for Record Access

Subject Access Request (SAR) can be made by an individual together with proof of identity. A SAR must be made in writing and the response will be within one month of the date received. There will be no charge for this information.

A SAR from a third party agent acting for the patient must be accompanied by the patient’s written consent for release of medical records.

You are entitled to a copy of the personal information we hold about you and to have any discrepancies corrected, this can be undertaken in writing to the above address at the top of the policy.

 

Disclosure of your information:

With your permission we may pass your information to other medical professionals who may be involved in your care, this may include: Consultants, Gp’s, Care staff, Multidisciplinary team members or other Health and Care professionals.
